How Does Home Health Care Work?
Tandem Health Services Inc. provides an in-home assessment to the patient. This allows us to identify, recognize, and understand the needs and situation of the patient at home. We will also be working closely with the patient’s family and physician in order to ensure that we are able to provide the appropriate care.
Our care coordinator will then come up with an individualized care plan that is suited to the daily healthcare needs and requirements of the patient. The individualized care plan will be updated from time to time to fit the patient’s changing needs.
